Out-Patient Center
The hospital’s out-patient department is composed of accredited consultants and physicians. The department has multiple HMO partners which provide efficient plans to member patients. The multi-disciplinary approach of the hospital ensures patients that they get the best care possible through the wide range of well-trained health care professionals and consultants in the roster.
OUT-PATIENT CENTER caters to all clients from Mondays to Sundays, from 7:00 am-5:00 pm.

Admission
The hospital’s Admitting office has a team of accommodating and helpful individuals who help patients in facilitating their admission to the hospital. The admitting office is designed purposely to complete all the needed requirements in a quick and orderly manner to make the process easier for both the staff and the patients. The Admission Department is located at the right side along the hallway lobby at the ground floor.
General Admission
- Patient should present their doctors admission order sheet.
- Patient under HMO/Company should present their ID upon admission.
- The Admission personnel assigns beds according to patients’ choice, medical needs and census count.
- For patient under HMO/Co, their room will be base on their HMO/Co Card.
- Upon completion of documentation details, the hospital admission personnel will issue you a patient personal identification bracelet. Always wear the identification (ID) bracelet during your hospital confinement. DO NOT remove the ID bracelet until after you are discharged
- As soon as the room is ready, patient will be escorted to the Nursing Unit by the Nursing Assistant or Orderly.
- Nursing staff will advise you of the services available during your stay and the care and treatment you may require after discharge.
- Please inform the nursing staff if you have any special needs or questions.
Emergency Admission
Walk-in Patients without doctors admission order sheet or a doctor known to them may be admitted through the Emergency Room. The ER consultant will assign an attending physician if the patient does not have a personal physician of choice.
- Patient relative will be advise to process the patients admission at the admission Department.
- Patient relative should present the Emergency Admission Form provided to them by the ER Consultants.

- Updated Member Data Record (MDR) with the current employer
- From your employer:
- Duly signed Employer’s Certification in Claim Signature Form (Part II)/Claim Form-1 (Part IV) and/ER-2
- Certificate of Philhealth Contribution (At least 6 months prior to confinement/utilization)
- Individually Paying/Self-Employed
- Philhealth ID or any government issued ID
- Updated Member Data Record (MDR)
- Clear copy of Official Receipt/s covering at least 6 months prior to confinement/utilization
- Overseas Filipino Worker
- Updated Member Data Record (MDR) stipulating the payment validity period
- Clear copy of Official Receipt/s (optional)
Indirect Contributor
Lifetime/Pensioner
- Philhealth ID for Lifetime/ Pensioner
- Updated Member Data Record (MDR) stipulating as non-paying member
- Indigent/Sponsored
- Family Health ID Card with validity
- Updated Member Data Record (MDR) stipulating the validity period
Senior Citizens
- Philhealth ID for Lifetime/ Pensioner

For Qualifying Dependents
- Updated Member Data Record (MDR) wherein the name/s of the qualified dependent is stipulated.
- Spouse: Copy of registered Marriage Contract
- Children below 21 years old: Copy
- Parent 60 years old and above: Copy of registered Birth Certificate of Philhealth member plus copy of parent’s senior citizen ID.
Emergency Services
Emergency Room – Grace Med’s 11 bed Emergency Room is one of the most modern in the region. Staffed with well-trained ER nurses and an ER Med (a doctor specifically assigned to the Emergency Room), emergencies are better treated. Effective triaging practice means that patients are cared for based on the severity of their cases when they arrive at the ER. There is also an isolation room where patients with suspected infectious or communicable conditions are checked and quarantined. Immediate and appropriate care is given to all emergency cases and no patient is denied treatment. Medical personnel are very aware of the high stress levels in the ER and are trained to handle scared and anxious patients and relatives.

If you are a patient, family member or friend of a patient, the Hospital Chaplain is available to provide for your spiritual and emotional needs. Just advised

Out-Patient Pharmacy
Med Express – Grace Medical Center pharmacy offers quality but inexpensive medicines. With a full support from Universal Laboratory (UNILAB), the pharmacy dispenses only quality generic and branded medicines. Licensed pharmacists and pharmacy assistants are present to explain the medication dosage and proper administration of doses.
Business Center
Business Center – The hospital’s business center’s goal is to maintain efficient billing and invoicing processes throughout the hospital. A well-managed invoicing system minimizes human error regarding patients’ inter-department transactions and other matters regarding billing. Grace General hospital implements a transparency of information on queries about billing process. A simple and innovative system not only saves time, but also reiterates our desire to show care for our patients.
